These Terms govern use of this website only. They do not replace or modify any patient consent forms, financial agreements, notices of privacy practices, or other documents governing clinical services provided by the Institute.

Not a substitute for medical care

The content on this Site, including text, graphics, images, and any other material, is provided for general informational purposes only. It is not intended to be, and should not be treated as, medical advice, diagnosis, or treatment, and it does not create a therapist-patient or provider-patient relationship of any kind.

No relationship formed through this website

Submitting an inquiry or appointment request does not, by itself, establish a provider-patient relationship with the Institute. Whether such a relationship exists depends on the circumstances, including the Institute's acceptance of responsibility for providing professional care.

Appointment requests submitted through this Site are requests only. We will follow up to confirm availability, gather any additional information needed, and schedule your visit. Submitting a request does not guarantee a specific appointment time, date, or that services will ultimately be appropriate for your child.
